When we serve as your health care navigator and advocate, all communication is confidential and in full compliance with HIPAA regulations. The patient must sign appropriate releases to allow family members, loved ones and ReAssured Advocacy to receive protected health information.
Depending upon the needs of the person and family, we can:
- Arrange transportation to and from appointments, including attending the appointment, if requested.
- Provide written summaries of appointments for patient and family/loved ones
- Help the patient and family understand medical terminology, medical tests, treatments and medications to enable them to make informed decisions.
- Keep family members updated and informed about details regarding patients’ medical issues, physician appointments and doctor recommendations.
- Research, evaluate and coordinate support services including elder law, medical alert, senior move companies and senior housing options.
- Coordinate appointments, home care, testing and transportation
- Schedule medical services following a hospital stay, including follow-up appointments, rehabilitation, treatments and medications.
